Appointments and Services

Website information about services, providers, availability, and prices may change. Offers are subject to their stated eligibility requirements, expiration dates, and restrictions. Any cancellation, deposit, refund, or payment policy will be disclosed separately when applicable; these website terms do not establish a cancellation fee or refund entitlement. Sending an inquiry or appointment request does not confirm an appointment; an appointment is confirmed only when awakenMD confirms it with you. Any informed-consent forms, treatment instructions, or appointment-specific policies provided to you apply to the services they cover.

Email Communications

If you email us, we may use your message and contact details to respond and assist with your request. If you sign up for promotional email, we may send news and information about awakenMD services or offers. Promotional emails will identify awakenMD and include a mailing address and an unsubscribe method where required by law. You may unsubscribe at any time using the instructions in the message or by contacting us. We may still send non-promotional messages needed to respond to your inquiry or manage an appointment. Email is not guaranteed to be secure or confidential. Do not send urgent requests or sensitive medical information by ordinary email unless an awakenMD provider has directed you to use an appropriate channel.

Text Messaging

awakenMD offers two-way text messaging for appointment-related communications, responses to service inquiries, and client support. If you provide your mobile number and opt in, you agree to receive the types of messages described when you opt in. Providing a phone number alone does not enroll you in promotional texting. Promotional texts, including recurring messages sent using automated technology if offered, require a separate express written opt-in. You do not have to agree to promotional texts to request or receive AwakenMD services. Message frequency varies based on your appointment activity and conversations with us. Message and data rates may apply to messages sent to you and to messages you send to us. Check with your wireless carrier about your plan and any applicable charges. You may opt out at any time by replying STOP to an awakenMD text. We may send one non-promotional confirmation of your opt-out. We honor requests made by any reasonable means and stop messages as required by applicable law. You may opt in again later. Your Mobile Number

You confirm that you are authorized to use the mobile number you provide. Notify us if your number changes or is reassigned. Text messages may be visible to others with access to your device and are not a secure channel for sensitive medical information.

Privacy and Third-Party Services

Our Privacy Policy describes how we collect, use, and share information through the website, email, and text messaging. The website may link to or include services operated by third parties, such as appointment or social-media services. Those providers have their own terms and privacy practices, which apply to your use of them.

These Terms of Use are governed by California law, without regard to conflict-of-law rules. Any court proceeding relating to website use must be brought in the state or federal courts serving Santa Clara County, California, unless applicable law requires another forum. If a provision is found unenforceable, the remaining provisions remain in effect. These terms and the Privacy Policy are the complete terms for use of the website, except that any separate written consent or service- specific policy governs the subject it addresses.
